Forgive me if you are already familiar with the Puella Magi Madoka Magica series. Just in case you aren't, I'm adding a little explanation in the question. ^^;
Homura starts off the series with horrible eyesight and some sort of heart condition that renders her very, very frail. Indeed, she has been hospitalized to treat it at times. After she becomes a magical girl, eventually she uses her magic to fix these things. Moreover, it is of note that when a girl becomes a magical girl, her soul / consciousness / self is removed from her body and made into a soul gem. Therefore her body becomes something of a lifeless shell, animated only when the soul gem is nearby.
Based on the FAQ, obviously Homura would regain a completely human body. And if it still had the heart defect, I'm assuming the crystal magic would take care of it as per no more diseases. But what about the eyesight? Would that remain fixed or because it was fixed with another world's magic and is not, in fact, life threatening in any way, would her eyesight return to its default level of suck? Or does this fall under the 'handicaps are up to you' question? ^^;
